Sciatica During Pregnancy: What Chicago Women Need to Know

Sciatica is one of the most common complaints we hear from pregnant patients at Loop Spine & Sports Center. If you are pregnant and feeling a sharp, shooting, or burning sensation running from your low back through your hip and down your leg, you are not imagining it, and you are not alone. A growing number of Chicago women, especially those commuting into the Loop or spending long hours at a desk, deal with this exact issue in their second and third trimesters.
This article covers what causes sciatica during pregnancy, how to tell it apart from normal pregnancy discomfort, when chiropractic care is appropriate, and what a visit with Dr. Trottier actually looks like.

What Is Sciatica, and Why Does Pregnancy Trigger It?
Sciatica refers to pain that travels along the path of the sciatic nerve, which runs from the low back through the buttock and down the back of the leg. During pregnancy, several factors can contribute to this: shifting posture as the center of gravity moves forward, increased curvature in the low back, hormonal changes that loosen ligaments around the pelvis, and the growing weight of the uterus placing pressure on nearby structures. Any of these can lead to restricted spinal segments or added stress on the joints of the low back and pelvis, which in turn can irritate the sciatic nerve.
It is worth noting that not every case of leg pain during pregnancy is true sciatica. Round ligament pain, general pelvic girdle pain, and muscular strain can all mimic it. Part of a thorough evaluation is figuring out which one you are actually dealing with, because the right approach depends on the source.
Common Symptoms
Patients typically describe:
A sharp or shooting pain that starts in the low back or buttock and travels down one leg
Numbness, tingling, or a burning sensation along the back of the thigh or calf
Pain that worsens with sitting, standing for long periods, or certain movements
Discomfort that may intensify as pregnancy progresses and the baby's position shifts
If you are experiencing weakness in the leg, loss of bladder or bowel control, or numbness in the groin or inner thigh, these are red flags that require immediate medical attention and are outside the scope of chiropractic care. Please contact your OB or seek emergency care right away if any of these occur.
When It Is Safe to See a Chiropractor
At Loop Spine & Sports Center, Dr. Trottier cares for prenatal patients after the first trimester. This is a standard, conservative approach that allows the earliest and most sensitive stage of pregnancy to pass before introducing chiropractic adjustments. Every pregnancy is different, so if you are unsure whether care is appropriate for your specific situation, a conversation with your OB and a call to our office is a good starting point.
Chiropractic care during pregnancy is not about forcing anything. It is a gentle, modified approach focused on improving mobility in restricted areas, reducing compensatory strain, and helping your body accommodate the physical changes of pregnancy more comfortably.
How Prenatal Chiropractic Care Can Help
Dr. Trottier's approach to prenatal chiropractic care is built around evaluating how your pelvis, low back, and hips are functioning together. When one area is restricted, another often compensates, and that compensation is frequently where sciatic irritation shows up. Pregnancy-specific techniques and positioning are used throughout, including supportive pillows and tables designed to keep you comfortable while lying face down is no longer an option.
The goal is straightforward: identify where movement is restricted, address the compressed disc space or decreased spinal curves contributing to the irritation, and help your body move and function with less strain as your pregnancy progresses. This is not a guarantee that every case resolves quickly. Some patients notice a difference within a few visits, others need a longer course of care, and a small number have underlying issues that require co-management with their OB or a referral to another provider. An honest evaluation at your first visit will give you a realistic picture of what to expect.
Massage Therapy as a Complement
Many prenatal patients pair chiropractic visits with prenatal massage therapy. Our licensed massage therapists use pregnancy-safe positioning and techniques aimed at reducing muscular tension around the low back, hips, and glutes, areas that are commonly tight and contributing to sciatic symptoms. This is not a substitute for chiropractic evaluation, but for many patients it works well alongside it.
What to Expect at Your First Visit
If you have never been to a chiropractor before, our post on what to expect at your first chiropractic appointment walks through the process in detail. For prenatal patients specifically, the exam is adjusted to account for pregnancy: certain orthopedic tests are modified or skipped, and X-rays are not taken. The consultation focuses on your history, your symptoms, and a hands-on evaluation of how your spine and pelvis are moving.
Sciatica vs. General Low Back Pain
Not every ache in pregnancy is sciatica. If your discomfort stays localized to the low back without radiating down the leg, it may fall under general low back pain rather than true sciatic nerve involvement. You can also review our full sciatica page for a broader look at causes and care outside of pregnancy. The distinction matters because it helps guide what kind of evaluation and care makes sense for you.
A Few Honest Notes on Scope
Chiropractic care can help many pregnant patients manage sciatic symptoms, but it is not a cure-all and it is not appropriate for every case. Some pregnancies involve complications where chiropractic care is not recommended, and some symptoms point to issues that need to be managed by your OB first. Part of a good evaluation is recognizing when a case falls outside what chiropractic can responsibly address and saying so directly, rather than promising results that may not be realistic.
